IU School of Medicine and IU Health have partnered with the Healthcare Professional Well-being Academic Consortium (PWAC) and an independent survey administrator, SullivanLuallin Group (SLG), for the 2026 Well-Being Survey.
We are committed to protecting the confidentiality and security of all the information you provide through this survey. SLG will conduct the survey through ServiceSTAT, a proprietary online survey administration, database and reporting tool. ServiceSTAT is a cloud-based tool for creating and administering online surveys. The database will be stored on a password-protected, encrypted computer system that is HIPAA-compliant, has limited access and is in a locked office within a controlled facility.
How We Protect Your Information
Only anonymous, de-identified data are shared with IU School of Medicine and IU Health leaders. All personal information, such as names, email addresses, employee IDs and NPI numbers, is removed from the respondent dataset and reports. Your personal information associated with this survey will not be available to any Indiana University or IU Health employee and will be accessible only to authorized personnel at the third-party data steward.
IU School of Medicine and IU Health will adhere to the laws and policies in place to protect the confidentiality of your personal information. Your personal information will not be shared with any Indiana University or IU Health employee. Your anonymized information will become part of a database that will be used to generate aggregated results and survey reports. Identifiable data will not be released for any reason. Data released by SLG to IU School of Medicine or IU Health for any approved purpose will first be de-identified by removing names, email addresses and any other unique identifiers. Summary results with fewer than five respondents will not be reported in any format.
Employee email addresses are used only by SLG to confidentially link responses over time and are removed before any data or reports are shared with IU School of Medicine or IU Health.

How We Use Your Information
Your confidential responses to this survey will help IU School of Medicine and IU Health leaders evaluate previous interventions, inform new local and enterprise-wide improvement efforts and guide the deployment of resources. Only a de-identified dataset will be shared with an approved IU School of Medicine and IU Health project team.
Survey data will be used for the following purposes, as permitted or required by law:
- Improving quality of the work and academic environment for clinical faculty, advanced practice providers, residents and fellows
- Guiding program and strategy development aimed at supporting professional well-being
- Helping to secure and justify funding for programs aimed at fostering well-being for the school and academic health center
- Tracking changes in wellness variables over time
- Assessing established programs and improvement strategies over time
- Contributing to national benchmark norms against which sub-groups can be compared
- Supporting secondary analyses that inform evidence-based practices and advance scholarship
For the purposes described above, survey data may be combined with data from other respondents and participating organizations and analyzed in aggregate. Results will be reported only in summary form, and no personally identifiable information will be included in any reports.
